Unique Headgear found in Diablo and Hellfire. There were no unique helms added in Hellfire. All of the following are found in both games.

The various pieces of unique headgear are among the most useful unique items to be found in Diablo, and several pieces can be found among the end game gear for a variety of character types.

Diablo Unique Helms

Harlequin Crest

This quest item is only found in single player. It's obtained from the innkeeper as a reward for returning the baner in the Ogden's Sign quest.

The hat's name is familiar to most players since it was used for the similarly-themed, hugely-overpowered unique Shako in the Diablo II Expansion.

Thinking Cap

The most popular helm for many mages, the Thinking Cap is unusable initially, with just 1 durability. Players who wish to use it must do "shrine runs" in the Church or Catacombs, looking for a Hidden Shrine which may boost the durability of all but one equipped item by 10. In Hellfire it's easier to fix, since Blacksmith's and other Oils boost item max durability.

Fool's Crest

An interesting mixture of huge hit point bonus and negative stats to counter it. No one wears this long term, but it can be a useful item during the mid-game.

Helm of Sprits

The name is assumed to be a typo, and the title should probably have been Helm of "Spirits." Despite the very limited bonuses, this is the only way to obtain life leech on anything but a weapon in multiplayer Diablo, and as such it's a very popular helm for warriors.

Unfortunately, it's enormously difficult to find one. Almost every monster that can drop a unique helm will instead drop the Fool's Crest or the Overlord's Helm. Due to the way the Mlvls and Qlvls interact, the best way to find one of these is to repeatedly run The Butcher on nightmare difficulty. The odds are still terrible, since how often does he drop a unique helm? But it is possible.

Overlord's Helm

A tolerable item for a warrior or a rogue who isn't much concerned with using magic.

The Undead Crown

This quest item is dropped by the Skeleton King and provides life stealing from the hat slot, something virtually no other non-weapons in the game offer. (The Skeleton King drops a random magical item in multiplayer.)

Royal Circlet

Probably the most popular unique in Diablo, this one was worn happily by Sorcerers, Rogues, and Warriors, who all enjoyed its big bonuses to mana, attributes, and the high AC.

Lightforge

This is a hacked item, and will never appear in Diablo or Hellfire. The graphic is on the CD, but it's unused, and the stats are partially from a weapon and partially invented. It's not an deactivated item, or a secret item; it's not in the game at all, other than as a hacked item players created and duped widely.

There's no telling how this one got started, but it became quite common in the hacks and cheats-infested Diablo 1 battle.net, and many players with these would swear they were legit, and that Diablo (or whoever) dropped it. Not the case.

Veil of Steel

This quest item is a reward from Lachdanan, and can not be obtained in multiplayer Diablo. It's a very useful item for a character who can afford the mana penalty, and even if you're not going to wear it, it sells for a fortune.

Gotterdammerung

A popular item with some builds, and exceptionally useful for melee fighters on levels without witches or mages. Often used unidentified, since the 60 AC is by far the highest on any helm. Reduces resistances to 0% regardless of what other items are equipped; even counteracts the Constricting Ring.

The name is German, and means "Twilight of the Gods." One of Wagner's best known operas used this title, which is a translation of "Ragnarok," the Norse religious version of Armageddon.

The helm is often referred to by Diablo players as "Goddamnitsdark," in humorous reference to the extreme light reduction it mandates.
