These odds apply to v1.10+ only.
The "Rarity" odds assume that the monster dropping the amulet is capable of dropping every unique ring in the game. Hence, if a monster able to drop Metalgrid drops a unique amulet, there is a 2/75 chance it will be the Metalgrid
If the monster's Mlvl isn't high enough to drop some of the amulets, the rarity odds change accordingly. D2C Amulet Generation Order
In Diablo II 1.06 and prior, there was a set order for the three rings and three amulets to be generated in, either from gamble or monster drop.
Nokozan Relic -> Eye of Etlich -> Mahim-Oak Curio
The game would always drop the Nokozan Relic unless it was already in the game. If it was, then the game would always drop an Eye of Etlich. Only if both those were in the game would a Mahim-Oak Curio appear. If all three were in the game and the game tried to create a unique amulet, either from a monster drop or from gambling, it would generate a rare instead. Since the Eye is the most popular/useful unique amulet, most characters would carry a Nokozan in their stash, or carry all three to increase their odds of a Rare amulet.
This order was abolished in D2X and D2 1.08, when drops became randomized, but weighted by the rarity odds listed above.
