Unlike tombs, prisons, or other man-made structures, the maggot lair is, obviously, dug by the Sand Worms and other maggot-like creatures. As a result, the passages are very narrow. The Maggot Lair can be found in the Far Oasis level of Act II. It contains the Staff of Kings for The Horadric Staff quest. It's three levels deep.
